  1. Please rate your level of satisfaction for each of the following park service areas on a scale of 1–10, with 1 representing the least satisfaction and 10 the most, by selecting the number from the pull down menu.

  2. Consider the bullet point questions in each section when making your evaluation.

  3. Please include comments with concise but specific information that will help guide park staff in addressing your concerns and knowing when they have met your expectations. Identify the location of the problems areas.
